I hereby waive and release PROKids Baseball Academy from any and all liability for any injury or illnesses occurring while my child is attending the Academy. I have no knowledge of any physical impairment that would be affected by the above named camper’s participation in the PROKids Baseball Academy as outlined in the brochure.

Hours: 9:00 am to 3:00 pm (8:30 am check-in, 3:30 pm pick-up)

Ages: 6yrs to 12yrs old

Cost: $250.00 per player (sibling discount available)

Includes: Camp T-shirt

Location: All camps held at Chapparosa Park, Laguna Niguel, CA
